Electrical & Plumbing Maintenance, Repair, and Operations (MRO) Supplies Reporting

General Report Requirements:
→Reports must be received on or before Due Dates.
→All reports are required on the 15th of each month even if there is No Activity. Indicate "No Activity" somewhere on the spreadsheet.
→(Reporting Period = purchases for previous month)
→Return completed reports to Brooke.Gullick@wisconsin.gov via email. Subject line of email should read; E&P MRO Monthly Sales Report - (reporting month)
→All data columns on the report must be completed accurately when spend data is reported.
→Fill out the Reporting Data (e.g., Full Vendor Name, Reporting Period, etc. on the spreadsheet.)
→All State Agency spend including all University of Wisconsin campuses must be included on the same worksheet. A separate worksheet contained within the monthly excel report shall be submitted for municipality spend.
General Report Format:
→Enter only orders issued the previous month. (i.e., Report due January 15th will have December invoice dates reported.)
→Enter only one order date and one order number per line.
→Add rows to the bottom of report as necessary.
→The "Quantity per Unit of Measure" column should be filled out with integers only (e.g. for a pack of 2, enter only "2")
→All pricing ordinarily includes shipping. If additional shipping charges were levied, enter them as
a separate line item.

Municipalities are also part of the Contract and are defined as any county, city, village town, school district, board of school
directors, sewer district, drainage district, vocational, technical and adult education district, or any other public body having the
authority to award public contracts (s. 16.70(8), Wis Stats.)and must also be included in the report.

Provide only one order number per line Provide date order was placed For each line item, list which subcategory (ie market basket tab; lamps/data cable, etc) the product falls under regardless of its Tier. If it doesn't fit in one of the tabbed categories, please indicate however it's commerically categorized.

Provide your name Provide your email address Indicate 'Y' for Yes, if a State Purchasing Pcard was used as method of payment. Indicate 'N' if No.


Name of Agency/University/ Municipality Customer Account Number



Use Acronyms tab to fill out name. For example of report sales for TAYCHEEDAH CORRECTIONAL INST, you would list them as DOC
Account number linked to the person who made the purchase.



Use all CAPS to keep the field uniform




The use of generics such as STATE or UW are not acceptable.




Keep naming convention for each municipality uniform throughtout life cycle of contract reporting; if reporting for BROWN CO HWY COMMISSION then always list as BROWN CO HWY COMMISSION,not occassionally as BROWN CO HIGHWAY COMM or any other variation.

Indicate which Tier the product fall into.
Tier 1 - if it is specifically listed on one of the tabs from the Market Basket categories
Tier 2 - if discount from manufacturer's price list
Tier 3 - if discount from contracter's price list
Consistent Naming convention is required. Provide Manufacturer SKU/Product # Maintain consistent description for product each time it's listed throughout reporting for the lifecyle of contract. Provide your contractor product number Indicate in what quantity the product is sold. IE EA (each), FT (foot), PK (pack)

Inidcate what your purchase price is for the product listed on this line. Inidcate the price the end user is paying for the product listed on this line. Indicate quantity of UOM ordered. You should not need to enter data into this field. This field should auto-populate once data is entered into columns F & G.
